Release, Waiver, and Indemnity

In consideration of the acceptance of the Application by my minor child (”My Child”) to participate in the programs and activities of the Inside Edge Hockey Schools Inc. (“School”), I, in my capacity as parent and/or legal guardian of My Child, for myself, all of our legal representatives, heirs, and successors assume all risks of My Child’s participation in the School and release, waive and forever discharge The Inside Edge Hockey Schools Inc., its sponsors, employees, instructors, officers, directors, suppliers, participants, volunteers, participating municipalities, heirs, successors, all other associated persons and their legal representatives (“The Inside Edge Hockey Schools Inc.”) from all claims, damages, costs, expenses, actions, in respect of My Child’s death or injury, loss or damage to My Child’s person or My Child’s or my property howsoever caused, arising from My Child’s participation in the School, including, but not limited to incidental contact in on-ice participation, whether as participant or otherwise, before, during or after the School and notwithstanding that same may have been caused by the negligence of any of The Inside Edge Hockey Schools Inc. 

I hold and save harmless and agree to indemnify The Inside Edge Hockey Schools Inc.  against any liability incurred by any of them arising as a result of My Child’s participation in the School. 

On behalf of My Child, I agree to comply with the rules, and regulations of the School. 
I warrant and represent that: I understand and undertake the risks of the School; and, that My Child is physically fit and has sufficiently trained to participate in the School. I acknowledge that there is no obligation on the part of The Inside Edge Hockey Schools Inc.  to provide medical care to My Child during or after the School. I consent to any emergency medical care provided to My Child in the course of attending the School. 

I grant permission to use My Child’s name, personal information (as that term is defined in the Personal Information Protection Act), voice and picture in any broadcast, advertising, or other account of the School for any purpose.
